CARE Network - HIV / AIDS

 

PERSONAL ASSISTANCE FOR PEOPLE LIVING WITH HIV / AIDS

The CARE Network's Case Management program supports and assists Napa County residents who are HIV positive or living with AIDS.  We are a speciallzed team of healthcare professionals and client advocates dedicated to finding what you need to continue living well.  Our services are free of charge, and are provided in English and Spanish.

 

Our case management team includes registered nurses and licensed social workers available to meet with you or come to your home to:

  • Advocate with healthcare providers for early intervention
  • Educate you and your family or caregivers about your health condition and necessary precautions
  • Explain nutrition and diet
  • Provide medical and psychosocial case management and support
  • Serve as a liason between you and your HIV / AIDS Physician Support as needed
  • Help you access health and social services resources and referrals

All information is kept confidential
