CARE Network - CANCER

 

PERSONAL ASSISTANCE FOR PEOPLE LIVING WITH CANCER

The CARE Network's Case Management program supports and assists low income residents of Napa County who have cancer and are under-insured or uninsured.  We are a speciallzed team of healthcare professionals and client advocates dedicated to finding what you need to continue living well.  Our services are free of charge, and are provided in English and Spanish.

 

Our team includes registered nurses and licensed social workers available to meet with you or come to your home to:

  • Simplify your healthcare by coordinating various medical services
  • Educate you and your family or caregivers about your health condition and necessary precautions
  • Explain nutrition and diet
  • Complete a medical and psychosocial assessment
  • Consult with you on managing your medications
  • Serve as a liason between you and your doctor
  • Help you access health benefits
  • Assist with community resources and referrals
  • Provide emotional support for you and your family
  • Cancer support groups available
